  • Download the necessary form(s), print it out and fill in the required information.
  • Bring in the completed form to your 1st appointment or at boarding check-in. 
  • New Clients: Please make sure to bring a valid drivers license or state ID with you to your appointment as this is required.  
  • Clients must be 18 years or older
